One document, sent to one or more recipients, with its fields, its signing order, its audit trail and its certificate of completion. A ten-page contract signed by four people is one envelope, not four. Your monthly count is every envelope you create inside a calendar month — drafts included, because a draft holds an uploaded document — and it resets on the first.
No. Per-signature pricing is the reason electronic signing gets expensive at exactly the moment it starts being useful, so there is none here. You pay for a sender seat, and that seat collects as many signatures on as many envelopes as it needs.
Billing is handled by Stripe, and switching is done from Billing in your dashboard, which opens Stripe's own customer portal. Stripe prorates the change: moving to yearly mid-month credits the unused part of the month you already paid for against the $72 year.
The plan stays active until the end of the period you have already paid for, then the account drops back to the free tier rather than being closed. Completed documents, their certificates and their audit trails stay in the account and stay downloadable — cancelling a subscription is not a reason to take away a record somebody signed.

No, and there is no point at which they are asked to. A recipient opens a single-use link, signs in their browser on whatever device they are holding, and receives the finished document. No account, no app, no download, and no paywall in the middle of signing.
